Effective date: October 20, 2026
This Privacy Policy explains what information everwell360.com (the “Site”) collects, how Gunardi Irawan, MD (“the Practice,” “we,” “us”) uses it, and the choices you have. The Site is a public information website. It is not a patient portal, and it is not the channel we use for your medical records or clinical communication.
This policy and your medical records are different things. Information about your care as a patient of the Practice is protected health information governed by HIPAA and by our Notice of Privacy Practices, available at the office. This Privacy Policy covers only the Site.
Information you give us. The appointment-request form asks for your name, mobile phone number, insurance carrier, preferred days and an optional note. We use it to call you back about scheduling and for no other purpose. Submissions are sent by email to the Practice’s scheduling staff. Please do not enter symptoms, diagnoses, medications or other health details in the form; it is not a secure medical channel.
Information collected automatically. When you visit the Site, we and the service providers below receive standard technical information: your IP address, browser and device type, the pages you view, how you reached the Site (for example, from a Google search or a Google ad), and cookie identifiers. We use this to understand how the Site is used and to measure whether our advertising brings people to the Site.
Cookies and Google tools. The Site uses Google Tag Manager, Google Ads conversion tracking and related Google services. These set cookies and may record that you visited the Site after clicking an ad, called the phone number shown on the Site, or submitted the appointment form. Google’s call tracking may display a Google forwarding number in place of our office number so that calls from ads can be counted; the call still reaches our office. Google’s use of this data is described in Google’s privacy policy at policies.google.com/privacy, and you can control ad personalization at adssettings.google.com. You can also block or delete cookies in your browser; the Site will still work.
Other service providers. The Site runs on WordPress hosted by Namecheap, Inc. (United States). Patient reviews shown on the Site are republished from Google through Trustindex. The accessibility widget is provided by UserWay. The Patient Gateway link goes to a portal operated by Mass General Brigham under its own privacy policy. Each provider receives only the technical information needed to deliver its service, and none receives the contents of your appointment-request form except our email provider, Google (Gmail), which delivers it to us.
How we use information. To respond to appointment requests, to operate and secure the Site, to measure the Site and our advertising, and to comply with law. We do not sell personal information, and we do not use Site information to make decisions about your medical care.
Sharing. We share information only with the service providers above, when required by law or legal process, or to protect the rights and safety of the Practice, its patients or the public. We do not share form submissions with advertisers.
Retention. Appointment-request emails are kept for 12 months and then deleted, unless you become a patient, in which case scheduling notes become part of your record. Technical and advertising data is retained by Google under its own retention settings.
Security. The Site uses HTTPS encryption, and access to the WordPress administration is limited and password-protected. No website or email system is perfectly secure, which is why we ask you not to send health details through the Site.
Children. The Practice treats adults 18 and over. The Site is not directed to children, and we do not knowingly collect information from anyone under 13.
Your choices and rights. You can use the Site without submitting the form. You can ask us what information we hold about you from the Site, ask us to correct or delete it, or ask us to stop using it for advertising measurement, by contacting us below; we will respond within 30 days. Residents of states with consumer-privacy laws (for example, California) may have additional rights, which we will honor as those laws apply to the Practice.
Changes. We will post any changes here with a new effective date.
