This website, everwell360.com (the “Site”), is operated by Gunardi Irawan, MD (“the Practice,” “we,” “us”). The Site describes our primary care and medical weight-management services in Northborough, Massachusetts, and publishes general health information. Please read this page before relying on anything on the Site.
The Site is general information, not medical advice. Articles, pages, answers to common questions and other content on the Site are provided for general educational purposes. They are not a substitute for a consultation, examination, diagnosis or treatment by a licensed clinician who knows your history. Do not use the Site to diagnose or treat any condition, and do not stop, start or change any medication or treatment because of something you read here.
No doctor-patient relationship is created by using the Site. Reading the Site, sending us a message, or submitting the appointment-request form does not make you a patient of the Practice and does not create a physician-patient relationship. That relationship begins only when you have been accepted as a patient and seen by one of our clinicians. Until then, we have no duty to follow up on anything you send through the Site.
In an emergency, call 911. If you think you may have a medical emergency, call 911 or go to the nearest emergency department immediately. Do not use the Site, email or the appointment form for urgent or emergency matters; they are not monitored around the clock.
The appointment form is not a secure medical channel. The form is for scheduling only and asks for your name, phone number, insurance and preferred days. Please do not include symptoms, diagnoses, medications or other health details in the form or in any email to the Site. Existing patients should use the Patient Gateway portal or call the office at (508) 770-6701 for anything clinical.
Weight-management results vary. Descriptions of the EverWell program, including any discussion of medications used for weight management, are general. Individual results depend on many factors and are not guaranteed. Whether any medication is appropriate for you can only be decided by your treating clinician after an evaluation, and nothing on the Site is a prescription or a recommendation for any individual.
Content reflects the date it was written. Medical knowledge and guidelines change. Articles are dated, and we do not update every article as guidance evolves. Check the date and consult a clinician for current advice.
Reviews and testimonials are individual experiences. Patient reviews shown on the Site are republished from Google and reflect those individuals’ experiences. They are not a promise of any outcome for you.
Telehealth. Telehealth visits are offered only to established patients located in Massachusetts at the time of the visit, and only where clinically appropriate.
Third-party links. The Site links to other websites, including the Patient Gateway portal and Google. We do not control those sites and are not responsible for their content or privacy practices.
